Updated the remaining Snomed codes from the discussion with Dylan and Carlos

Overview

This pull request aims to update the SNOMED codes across various Synthea modules which came from the meeting we had with Dylan and Carlos, ensuring they are current and active

Hi @florim14 , apologies once again for the delay in responding to this. I had a chance to review these changes with our project lead, and we're good with everything except the changes to these dental modules. Turns out this code 456191000124101 used a few times in this module and once in "dentures" was selected specifically for a certain group so we're hesitant to change it and affect them. I don't want to hold the rest of this up and make you rework it again, so I'm going to see if I can revert just these 4 instances and get this merged today. If I can do that I'll add "133899007" as a second code which hopefully helps the original issue
