An agentic system to automatically populate a doctor's referral form that allows a patient to access specialized care. This system is a proof-of-concept which generates a doctor's referral form based on patient-doctor conversation transcript and a doctor's clinical note.

Usage

This system requires access to OpenAI's API, which requries an API key. An API key can be generated on their platform here: https://platform.openai.com/docs/overview

Please add your API key to config.py

To install the required libraries for this system please run the following command:

pip install -r requirements.txt

To run the entire system use the following bash command

Note: if permissions are denied, then try; chmod +x main.sh

./main.sh

To run the data generation script use the follow command:

python data_gen.py

To run the extraction script use the follow command:

python extractor.py

To run the evaluation script use the follow command:

python evaluate.py

Format

This system populates this referral form in question from Markham-Stouffeville Hospital for their Centre of Respiratory Health Referral.

This form contains fields that will either be autopopulated by a doctor's computer system such as patient information like Name, Address, Health Card Number. Some other fields would be filled in by the Hospital Staff and thus are also not populated by this system. The fields that are autopopulated by this system are:
