Follow-up emails do not work with custom organizations

When I select "Yes, email me follow-up instructions" on a organization with custom name and email ("Can't find an organization? Click here to add one"), I do not get the "Your data deletion request to [...] has been sent" email, and I can thus not access a record of my request. Manually pasting the UUID of the CC'ed email into the URL leads to a 404 page.

The option seems to do nothing at all; if this is intended, isn't it better to remove the option entirely in that case?

Also it seems the domain is not filled in in the e-mail template.
