Diagnosis Code Voting model no longer used

Describe the bug - Required
The diagnosis_code_voting model was left in, but it is no longer used due to the refactoring in 0.15.
If this is removed, then I believe normalized_input__int_diagnosis_code_normalize should also be removed as that also would not have any references.
