Readme "Synchronize Database Scheme" - ormconfig.js

I am following the Project Setup but got stuck at "Synchronize Database Scheme". How does that work? Can you please make it more explicit? I am also wondering where I can find the ormconfig.js file. Do I have to create it on my own?
