Nested collection order changes?

Has anyone come across the collection tree changing?

It's happened a few times on a site I'm working on. I have a products collection with 179 entires. Sometimes I notice the nesting is broken. Entires that are supposed to be nested are moved to the bottom of the list?

This is a problem because it means the URL changes, plus the nesting shouldn't change on its own?
