[Bug] Deleted MCP server show on restart

Bug Description

If I've deleted some MCP servers using the CLI, while the ToolHive desktop app was stopped, then the deleted servers briefly show when I restart the ToolHive app.

Steps to Reproduce

1. Run an MCP server, say fetch
2. Quit the ToolHive app
3. Delete the MCP server using the CLI thv rm fetch
4. Restart the ToolHive app

Expected Behavior

I expect that the fetch MCP server won't show in my list of MCP servers.

Actual Behavior

The fetch MCP server shows up briefly (for ~2 seconds) in my list of MCP servers and then disappears
