Have a simple form to log an issue with a piece of equipment, will ask for your name, the equipment name (drop down perhaps) and details of the issue.
Will post in trustees and email trustees the details, possibly also in general or appropriate channel to advise of the fault.
(in lieu of a ticketing system, might be good to have it also open some kind of record that can be closed when the fault is rectified and send an update to the above that all is sorted).
