Get rid of menu "bounce"

Right now, the entire DOM loads before the navigation menus are actually added. The way around this would be, I think, to add a "dummy" external tool that would be the placeholder for the menu i the DOM (a là the Canvas-integrated tools, like Commons and ARC). Not sure it's worth the effort to have a not-really-there LTI as a placeholder, but it would eliminate the annoying menu bounce and slide.

Indeed, this could be done, as described here. I'm a touch worried about what happens with all the extra "fake" placements if the number of menus shifts… and there's no way to determine placement order within the overall global navigation menu.
