Add a "fork me on github" UI component

Could be interesting to highlight that this project is open source and that other can copie or modify it.

Also, every time I'm thinking of making a change to Elvis, I'll spend 5-10 minutes trying to remember what the repo is called. Would be helpful to have a quick link.
