Cannot add Siber 4 button remote, not all fields filled in

Summary

Cannot add "Siber 4 button remote"

An error appears saying not all fields filled in.
There is an "address" label seen but no input to provide one

I've attempted to add it without first connecting the ventilation machine.
I've immediately tried to find and connect to my "Siber 4 button remote" after connecting the integration to the bridge physically (serial).

Steps to reproduce

1. Connect the BRDG-02R13 physically though USB Adapter
2. Bridge is successfully added
3. (?) Put the "Siber 4 button remote" in pairing mode by pressing the two upper or two bottom buttons
4. Press Add accessory
5. Type a name
6. Choose "Siber 4 button remote"
7. Press Submit

@scabrero Any clue what is going on here? Seems like a UI issue, no logs are generated
