Describe the issue
Structured output is only allowed with default (Accurate) and parse_page_with_llm mode. If structured output is set no markdown is output, a structured output is returned instead. Accurate is not default anymore. which mode does a structured json output?

Accurate mode has been renamed to Balanced mode (since Premium has a better accuracy). It's the same as parse_page_with_llm.

Which one is the structured output referring to now? Accurate then?
