- School Name, Address, Contact Number, Email
- Books and stationery
- Student Uniforms (including undergarments, socks and shoes)
- there should be away to input details as per grade (male, female) and then there should be away to retrieve a summery with sizes for each item.
- School bags, water bottles, lunch box
- List students for each grade
- Total number of students, affected students
- Students with special requirements
- Damages to school (damaged items, type and severity of the damage, estimate if available)
- required items
- specific sections or labs (home science lab, science lab, computer room multimedia/communication room, dancing and music rooms). There should be away to find whats required for computer room or home science room, etc.
- Cleaning required or not? (check box)
- Water supply available or not? (check box)
- Power supply available or not? (check box) this required to clarify if they had power supply befoe the disaster.
- students who lost their parents.
All information should able to expert as spreadsheet, pdf.
