PDForm4 unhandled error on demo page

On the demo page (https://panoramicdata.github.io/PanoramicData.Blazor/pdform4) there is an unhandled error.
Steps to reproduce:

1. Go to the page
2. Click on the Reporting checkbox to enable it
3. Click on Source at the top of the page
   The browser shows "An unhandled error has occurred".

Alternative to reproduce:

1. Enter anything into the first or last name inputs
2. Click on Source
