The padding of the card header is different when the card has an icon set

Current behavior

For a card without icon set there is a padding set for the card header so the title and the actions would look like this:

For a card with an icon, there is no padding, and the action button's padding overflows the card when hovered over:

Expected behavior

The padding for the header of the card is the same regardless of whether it has an icon, ensuring a consistent layout.
