Support skipping based on blame.ignoreRevsFile #1222

Based on discussions from #1084 and #1125, two PRs are in order:

blame_ignore_revs_file = ".git-blame-ignore-revs" + filter_mono_commits_to_blame_ignore_file = true (both unexposed defaults)
filter_blame_ignored_revs = ? + one or zero exposed option (tbd)
