Create PR and Issue templates

There is no PR and Issue template currently in the repo. Because of this, the new PR's and Issue's description aren't in the valid format, and most of them are empty. 

We can create a PR and Issue templates by reusing them from https://github.com/opensource-nepal/node-nepali-datetime/.
