DUO MFA

I'm planning to setup MFA DUO for our Mirth Server , Can you please guide me on that.

There is a premium (licensed) plugin for that from Nextgen. Also there are some open source things you can perhaps consider from https://github.com/ChristopherSchultz/mirth-plugins/tree/main/mfa-auth.
