v5: How does auth() works with nextjs middleware limitations and cache concerns?

I see the examples don't make a distinction on using middleware even when on a DB backed authorization. How does that work?

Also saw that showing changes on layout doesn't have the Provider pattern. How well does that plays with the caching layer?
