MPI 5.0rc review for pages 807-832 (MPI standard pages 761-786)

Several problems were found that would be nice to fix for the 5.0 release:

• in chapter 16 deprecated interfaces, all of the 'c' binding prototypes that are multi-line have messy (no) indenting
• The indentions for the prototypes for MPI_Copy_function and delete_function on pages 769-770 also is messy.

This indention problem has something to do with the behavior of mpibindmain in an itemized environment. this is beyond my latex skill level to fix at this point.

I had noticed this before but hadn't looked into fixing them. One easy option is to not use itemize for these. Alternately, I can look at inlining the appropriate macros in the itemize context, since these need to be handled differently as deprecated.

I will see about reorganizing the chapter without using items.
