Missing primary index and description of MPI_T_source_order

Problem

In reviewing the MPI 4.1 RC, I noticed that MPI_T_source_order doesn't have a primary index entry. In reviewing the document, this is because there is no discussion of this type, just a regular index entry macro.

Proposal

Follow the approach used for other MPI_T types and ensure that there is an mpictypemain entry for MPI_T_source_order.

@wesbland I remember this to be discussed as a Chapter Committee level change. Is this true? If so, we discussed this in the CC and think the PR would be ready for @wgropp to merge, so I would move forward on this, if this does not need to be presented at the next meeting.

Sorry for the super slow response. I agree that this is a CC change. I'll mention that on the PR.
