C++ gdb is not stopping or behaving as expected.

Outline: Windows, mingw64, g++, gdb, neovim.
Configuration files, logs etc below. I thought I had this working, but I introduced a <vector> and that resulted in 'failure to stop' issues. If it did stop, using the 'next' <F6> command resulted in a 'not focused' dialog etc. So I commented out the <vector> which seems to have permanently affected how the debug session works. The following is a session immediately after a re-boot - there are some errors with breakpoints in the dap.log.
Set breaks at lines 8,17 & 27. Start debugging with two arguments.
I would expect the break a 8 to be hit - it is not and the program continues to the first std::cin wait.
User input and the break at 17 is hit - but the 'DAP Scopes' is empty.
Stepping (F6) - the break arrow is replaced by the break symbol 'B' as if the program has stepped over - but it has not stopped at the next line of code.
Stepping (F6) again - a dialog appears 'Not focused on any stopped thread'
I would point out I've spent quite some time with this because the behaviour is not alway exactly reproducible. The session never really quite works, but sometimes some of the breaks and stepping works for one session, and reverts to misbehaving the next.
I suspect a timing/race condition - I've looked at the dap logs and in some instances the command to launch occurs before that to set breakpoint. But then I can't say I fully understand how DAP works.
Very happy to try anything suggested. In the meantime ...

I fixed almost all of the problems by reverting from gdb 15.1 to gdb 14.2.
