Provide a toy example metadata file and example results

Thanks for sharing the code for a great, interesting application.

In README you mention that you provide a metadata example file (.txt). I dont see any here in repo. It would greatly help if you can provide a toy example of multiple records with various ICD and ATC codes, and an example results after running the code as part of repo.
