Improve form validation

Certain validation rules should be added to the assessment form.

• if Sinkhole, a confidence level must be chosen.
• at least one basemap must be checked
• if Other, the comment field cannot be empty

The sink evaluation form is not currently operational, so these will have to be addressed when it is fully re-implemented.
