CREATE Table code for TUMOR table

The .sql code to create the tumor table can be useful without having to run the .jar stuff.

We have

https://informatics.gpcnetwork.org/trac/Project/attachment/ticket/749/tumor_data.sql

but that dates from March and the spec is dated May, so I'm not sure if that .sql code is exactly right.
