SRT Divider code

Hi Hayden!
I am extremely happy that my SRT divider code was useful to someone, but I would appreciate if you re-added the copyright owner notice line (which includes my name as the author of the code) to the Apache 2.0 license note at the top of System Verilog files.
It is important to retain the original copyright and license notice when reusing open source code to give proper credit to the authors.
