New Adds appear stuck in provisioning

Being very new to this product and also new to Home Assistant. My original z-wave stick failed - I lost all the associated data. So was some what starting from scratch. After adding in Z-Wave JS UI with the Zooz USB stick, I attempted to smartstart and add items to my Home Assistant z-wave system. However, after several attempts of items showing up in provisioning, but then nothing happening (they appear stuck), I shifted to Home Assistant Setup-Devices-Add New device. And via this process, I was able to add my light switches, etc. Yet, I still have several items "stuck" in provisioning. How do I remove them and perhaps more importantly, what may have gone wrong in the initial smart add process? The process of adding in Z-Wave JS UI into Home Assistant was flawless. All help is appreciated.
