If you have an atom open in multiple tabs, it is possible to set the atom to be embargoed in one tab, then publish in the other tab (or vice versa). The atom is then simultaneously published and embargoed, and at this point the atom's state is broken - no updates can be made, and the attached composer piece may not be publishable.
Having the atom open in multiple tabs does show a warning that edits may be lost.
We discovered this from user reports - I'm concerned that the state might have been entered from other means? Otherwise, it seems that the state can be entered by accident fairly easily.
