Access Denied coming for Embedded App

Hello All,

I am working on creating the Embedded Shopify App and after successfully installing the App I am just redirected to Access Denied Screen within Shopify.

I tried disabling the non-embedded option but in that case it just loads the token.blade view and nothing happens after that.

Okay, I debug it more and it seems the Token is not getting generated at the time of installation of the App.
I printed the redirection code within token.blade and below is the output
/?host=b25lLXRyYWtlci5teXNob3BpZnkuY29tL2FkbWlu&token=${token}

I checked the DB as well and can see the entry within user table but i don't see anything within personal_access_token table.
