GitExtentions toolbar is not show in Visual Studio after installation

There are a couple of issues with the default settings (in my experience)

1. After installing gitExtentions vsix - the GitExtentions toolbar does NOT become visible by default
2. In almost all cases that I did enable it manually, by right clicking on the toolbars and checking the GitExtentios, I do get a duplicated view (with all the buttons being show 2-wice)
3. (not sure if this is for everybody, but I need to enable this toolbar manually every time i start my VS :(
