How can I add a Nonce Manager to a ECDSA signing algorithm using NodeJS and Ethers.js?

I need to sign message with ECDSA with this parameters:

1. Address of user;
2. Premium sum;
3. Nonce by user (taken from smart contract, increases every time you pay and attached to a user)

I founded that in Ethers.js exists Nonce Manager, but I don't understand how can I use it in this case.

Can anyone help me with example of how to use it?
