Sending ether to an EOA (Externally Owned Account; an account controlled by a private key) doesn’t create an event.

An event only occurs when a contract uses one of the log* opcodes, which places the info in a bloom filter which gets included in the block.

So, there is, in general, no way to listen for simple ether transfers between accounts.

They probably listen for the "block" event and check for balance changes.

They may also run their own custom endpoints, similar to the API that Etherscan runs, so they can also detect internal transactions.
