index the needed information to understand and implement the CDE model in one place - eliminate the readme and repos maze please

Maybe this issue is for tooling independent implementors (the ones not (yet) using FAIR in a box).
Please make sure all needed info to implement the model can be found in one place, or at least indexed in one place so that users can find it easily.

Each model from the wiki is linked to this Markdown file so the documentation is completed and easily accesible for every data element. Exemplar data is also located in every data element wiki page
