Regarding with breaking change, wouldn't changing the comma to semicolon avoid this entire problem?

That comes with its own forever problems like the meaning changing if you miss a very subtle character difference.

Yes - the meaning will change if a comma is used and there is a method named with (or args or whatever - they keyword doesn't matter) - an extremely unlikely situation.

Maybe this was considered already, but what about just [(comparer); item1, item2]?

It was considered, but it's also just very easy to miss.

I would in general say that the with syntax is not fully decided. That includes whether a different divider will be used.
