Object ID is incrementing. How to keep it static for a single object?

Hello, while annotating a video the ID of the track in incrementing for every frame. How do I keep this static?
Ex:

and so on.. The track id for a single label should be static, but it's been incrementing for every frame we annotate.

Also, why am i getting duplicated rows? Here is what I see:

Help me with changes/precautions that I need to take so the output looks like this:

Make sure you are using exactly tracks for annotation, not shapes.
