Update to CQL to remove incorrect singleton pattern (STU3 and R4)

There is a singleton pattern being used that is not correct", ex: singleton from NoACEIOrARBOrARNIOrdered.reasonCode in "System Reason"
in 'Medical Patient or System Reason for Not Ordering ACEI or ARB or ARNI Outpatient"
of EXM135.
Find all instances of the pattern in pilot/connectathon measures and replace it with correct CQL.

Issue shows in STU3 measures- 104, 105, 108 and 71. None showing in R4 at this time. Add as issue in STU 3 draft measures repo. Will not pursue fixing specific CQL at this time.
