Entity (custom icon) toggle not functioning

Not sure if this is my misconfiguration, a Streamdeck issue, or a plugin issue.

The button in question (an "Entity: custom icons" button) is functioning just fine as an HA toggle to control a light. However, the two dots representing differing images for the toggle state do not appear to be working. Note that these two images have been "Set From File."

The light toggles just fine, and two different images are set (one for each 'dot'), but the Streamdeck does not flip-flop between the two images.

Frustratingly, I had this working in the past but now can't get it to work.
