Feature: detect and handle provider removal/retirement #19
Description
In the case of NY, since we have stable integer IDs, we don’t need to worry that the omission of a provider could be because of a rename.
So: if a provider is omitted from N sequential updates, then we should probably mark it as retired in the database.
