This was submitted by sean and failed validation. The datum value makes ellipsoid height and unit required. This passed client side validation and failed server side validation.

@seanfernandez do you have anything to add since this happened to you?

What did the app do when this failed? Did it make you aware that it failed

Honestly I'm not sure when it happened. I was preparing for the live presentation and may have been a little too clicky and didn't notice.
The only take away I have from the presentation is when the form completed it said Beaver County at the top and the sheet was created in Duchesne County. I was under the impression the BLM point ID was used to determine which form template would be used.
I'll work with Nathan to test further and submit an issue. There was also a little confusion on the units, but I'll also verify and submit an issue if needed.
