Align export options displayed on Appearance page and in the Editor #724

I recently finished writing up a tut about child themes, using the Create Block Theme plugin. I noticed some inconsistencies in > the interface options. It would be nice if we could clean up these UI inconsistencies to make a smoother UX.
CBT page - If you go directly to the CBT page (via Appearance > CBT), you arrive on the CBT page and are given 4 options.
Then, after choosing to create a child theme, there are 4 modal fields to fill out.
CBT panel - If you go to the CBT panel (via FSE: Appearance > Editor > Design > Styles > CBT), there are 8 options.
Then, after choosing to Create Theme, there are 2 options (Clone or Child Theme). THEN, there are 5 modal fields, 4 of which are grouped together under the Additional Theme Metadata area.
