Replace scenarios for END FORM HERE in D1a js

Replace D1a jquery js with UI Behaviors for the 3 scenarios to **END FORM HERE**:

1. When `SCD` = 0, remaining inputs should be disabled.
2. When `SCDDXCONF` = 1, remaining inputs should be disabled.
3. When `SCDDXCONF` = 0, remaining inputs should be disabled.
