define editors and authors

As discussed initially, we want to separate out the authors listed here as editors (Erika, myself, Bill, Norm) and regular authors (probably by chapter). Obviously Alyssa, + whoever else has contributed. We can discuss any questions on this in person.

based on our conversation, for now we will:

• have just authors for book
• add author contributions section at the end
