A couple trees with mortality in the notes

@ValentineHerr This is a small issue but I had a couple trees that needed mortality but didn't have the mortality form. For tree 60134 I think it didn't have the mortality form because it was incorrectly labeled DC in last year's mortality census, and tree 40150 might have just been missed. I took mortality data for both in the notes section and I was wondering if that could be added into the database somehow? Thanks so much!

Tree id 102316 was not found in 2018 but is actually big enough for mortality, none was taken in the notes but I still wanted to add it in here
