Modify the visit summaries in the patient chart

In the "Encounter" tab of the visit summaries in the patient chart, remove "Lab results" and "Notes" encounters as they are already available in the "Tests" and "Notes" tabs.

Also, rename "Encounters" tab by "Consultation".
