[Bug] 'Add Doctor' form Non-functional and overflowing #575

Is there an existing issue for this?

Bug Description?
The form is overflowing out of the screen and also upon clicking the 'Add doctor' button, nothing happens. On the technical side this happens becoz there isn't any backend route for the post request being made.
