Ensure `decimal` being the same as `delim` is handled consistently

When decimal is the same as delim, decimal should only appear in quoted fields, outside of quoted fields that character should be interpreted as delim. Same as we do for groupmark (#182) and similar to how we handle ambiguous or special characters in strings
