Problems with value types and slices

We really like to use this extension in our profiles, But we see two main problems with the profile.
The first problem is,
the value type has not been defined and the value type should be String since the middle name is a String type.

The second problem is, why do we need to slice this?
Can't we create a simple extension as shown in the second screenshot?
Due to these slices, we get following errors(in the third screenshot) with the latest (R4)fhir profile validator.
