Show summary information of configured providers in Feature Health tab of feature slide-out

In the 'Feature Health' tab of the feature slide-out modal, we should show some summary information of any configured providers. The key information that I would want to see:

1. The name of the provider
2. The webhook URL (perhaps just as a 'copy to clipboard' option rather than showing the whole URL
