Correction to Lung Cancer mappings

Hi,

I had now time to address the wrong mappings related to lung cancer.
Basically there are two things:

1. Squamous cell carcinoma is mapped incorrectly (mapped as mucoepidermoid) -> provided new mappings in the file
2. Bunch of lung surgeries are not mapped. Not sure how to proceed with them, and what is the correct codes to map them to

Marianna: The no 1 we can correct, see "Diagnoses, wrong" ie first 7 rows in Alex's excel:

The no 2 would need medical expert for mapping.
