Add possibility to fill dynamic pager for notifications #20
Description
Add to base notifier a simple form to fill database with somes contact could be notified by sms sending.
Typicaly, define a generic contact with special pager number in nagios contact. When this contact was notified, notifier could get information from database to extract contact pager and send sms to them.
